Insurance Information
At MAIA, we accept most insurance plans. Take a look at the list below to ensure your exams and treatments will be covered:
- Aetna PPO
- Blue Cross/Blue Shield PPO
- BlueCare Direct
- Blue Precision
- Cigna PPO
- Coventry
- Devoted HealthCare
- First Health
- Humana
- HealthLink/Unicare
- HFN
- HST Care Connect
- Medicare
- Multiplan
- PHCS
- United Healthcare
- BCBS HMO Illinois (your PCP must be with Advocate Good Sam)
To ensure your visit is covered, first schedule an appointment. After your visit, we will submit a claim to your insurance company. Finally, wait for feedback from your insurance company to see what will be covered and what you’ll need to pay out-of-pocket. This typically takes 1-4 weeks.
Whether you have insurance or not, we require every patient to have a credit card on file.
Once we receive the statement from your insurance company, we will notify you in writing through our secure patient portal with a copy of the insurance statement. We will then charge the credit card for anything insurance didn’t cover 24 hours after your notification. Credit cards are stored with Stripe, and their privacy and safety are guaranteed.
If you do not wish to keep a credit card on file, you must pay for your visit in full on the day of service.
Coverage for Annual Gynecological Preventative Visit
Insurance typically covers an annual preventative gynecological visit, which includes:
- Breast and pelvic exam
- Pap smear, HPV screening, STD screening
- Mammogram referral
- Prescription refills
- Discussion of birth control options
- Blood pressure, discussion of weight/diet/exercise, etc.
It does not cover any discussion of any problems or issues requiring tests, imaging, prescriptions, referrals, or a lengthy discussion time. You can be billed for both a wellness visit and a problem visit during your annual.
What if I don’t have insurance?
For patients who do not have insurance, we do have self-pay options available and can provide pricing upon request.
